Travel Permission Form


 

J.M. Hanks High School Band

 

My son/daughter has my permission to attend, travel to and from, and participate in:

  1. All activities related to the Hanks High School Band occurring on any Ysleta ISD approved function.
  2. All activities related to the Hanks High School Band that might involve travel to various places identified as appropriate by the Directors i.e. other stadiums, colleges and high schools.

My son/daughter has assured me that he/she will conduct himself/herself in such a manner that good credit will be reflected upon the school.

I understand that students will be chaperoned while on any school trip.

Both my child and I understand that all Ysleta ISD policies and guidelines are in place during any band event regardless of its location or time.

Medical Release

I authorize a YISD representative:

  1. To represent me before any medical institution where it may be necessary to send my son/daughter while he/she is under their care.
  2. To give in my name the necessary authorization for surgery in case of emergency, when medical authorities deem it indispensable.
  3. To represent me while my son/daughter is under their custody and control.
